There is a new version available for download. This has some cosmetic changes to a couple of the forms and also the tab order for the Code and Reference forms has been made sane. It was just a little bit to infuriating before.
There is also a new contact us page - where you can send in thoughts or comments about the application. This uses the Google Sites/Documents approach to manage form submissions.
